Cutting Errors in Total Knee Replacement: Assessment by Computer Assisted Surgery

چکیده

The observed errors in the position of the implanted prosthesis can be due to a number of potential causes. One of these is the potential error during execution of the bone cuts. However, there is only minimal information on this in the current literature. The amount of cutting errors in forty consecutive total knee replacements was reported. All the operations were done by the same surgeon. The amount of cutting error was measured by the use of computer navigation system. It was hypothesized that there was no difference in the amount of error between bone cut through the cutting slot (slotted cutting) and bone cut done on the surface of the cutting guide (open cutting). It was found that the average absolute cutting error was 1o in the coronal plane and 1.4o in the sagittal plane. Significantly more outlier (more than 3o) was observed in the errors in the sagittal plane (p=0.014, chi square test). Open cutting resulted in less error in the sagittal plane of the tibial cut when compared with slotted cutting (p=0.031, Mann-Whitney U Test). This was attributed by the use of a thicker saw blade with higher stiffness in the open cutting method.

Distal Femoral Valgus Cut Errors in Total Knee Replacement

The causes of malalignment in total knee arthroplasty can be categorized into three different groups; 1) Errors in bone cuts 2) Errors in implant fixations, and 3) The method of setting down the cutting guides (1). We would like to announce that more several distal femoral valgus cut errors may occur during total knee replacement.

Radiologic Assessment of Distal Femur Cutting Angle in Varus Knee Candidates for Total Knee Arthroplasty

Background: In a total knee arthroplasty surgery the goal is to produce 90 degree angle between the knee articular lobe and the mechanical femoral line. Most orthopedic surgeons usually utilize a 5 to 7 degree for distal femoral cutting angle. In this study we will aim at clearing this question, that whether the” five-seven degree” distal femoral cutting angle supposed to be an equable spectrum...

Use of the Cusum technique for evaluation of a CT-based navigation system for total knee replacement.

Most of the early failures of total knee replacements are related to technical flaws. Conventional ancillary devices achieve good alignment in the frontal plane in only an average of 75% of total knee replacements. Computer-assisted surgery may improve the technical quality of implantation surgery. The aim of our study was to evaluate the use of computer-assisted surgery using a quality control...

Embolic phenomena during computer-assisted and conventional total knee replacement.

Systemic emboli released during total knee replacement have been implicated as a cause of peri-operative morbidity and neurological dysfunction. We undertook a prospective, double-blind, randomised study to compare the cardiac embolic load sustained during computer-assisted and conventional, intramedullary-aligned, total knee replacement, as measured by transoesophageal echocardiography. There ...

Joint line position correlates with function after primary total knee replacement: a randomised controlled trial comparing conventional and computer-assisted surgery.

The role of computer-assisted surgery in maintaining the level of the joint in primary knee joint replacement (TKR) has not been well defined. We undertook a blinded randomised controlled trial comparing joint-line maintenance, functional outcomes, and quality-of-life outcomes between patients undergoing computer-assisted and conventional TKR. A total of 115 patients were randomised (computer-a...
